It's appearing more and more that my trip in late may will need to be cancelled, this stupid virus can go away any time now. I should be scuba diving with some sharks at this moment ?

I'm actually surprised that in all of this chaos, that no one from the leadership level put out a piece on the news or something regarding mask etiquette and etc. People are hawking the news stations, so it seems it would be rather easy. I'd do it for free

Ah gotcha, that makes sense. As far as masks, bandanas/cloth are OK and the combination of 2 people wearing something actually significantly decrease chance of transmission.... Even N95 aren't 100% and if they fit/worn wrong are actually not that protective. It's true the point of most masks is to keep your stuff out of the general publics stuff. I still see a ton of people in their cars alone driving with a mask on, must be really uncomfortable

New York now just said that if going in a public place where you can't social distance you are mandated to wear a mask (I heard this from a friend, so idk the specifics) but, shouldn't this have been a thing from the start, seems weird that the timing comes after NYS is on the "downtrend" I feel the people making these calls just simply don't have a clue, can't wrap my head around decisions simply from a common sense standpoint.

I think that's actually quite appropriate, it's easy to use, doesn't hurt supply for those in Healthcare and serves a basic purpose....plus people look a lot less stupid with a bandana on their face than an inappropriately worn medical grade mask.

If I see someone wearing a general surgeon mask wrong I chuckle.... I get very annoyed when I see the general public wear an N95 mask to begin with, when I see it improperly worn then it bothers me.... Those should be in our hospitals..... But instead we get bs lip service of being "heroes" while still getting treated like ***** by the general population

People suck, I am up in the andirondacks and this morning there was an older gent walking into the Stewart shop with an 95 mask on, but he had the straps all jacked up, the mask was basically useless. I offered to help him fix it, I even alluded to the fact that I'm in health care and he was just a ride @ss. He said he knows what he's doing and he doesn't need people correcting or critiquing him..... I hope he gets it.... I'm serious.

I have fairly high confidence that once infected, even mildly, that you have a very good amount of immunity, 99% and up..... It's simple immunity and pathophysiology and follows similar science to other viruses of this class. This is just gut speak..... Now onto the point of length of immunity and if you can still transmit, those are definitley questions that we need to find solid answers to as those are a bit more of an unknown. I think the incubation period is the real issue here, once you have clinical symptoms and those resolve, my best guess is that your chances of spreading the disease drop significantly. There are too many theories floating around out there, a lot of people want to play hero, some may be right, but many are just speculating.... Kinda like me ?

Damn, isn't humility one of the main teachings in many religions? Politics and religion are just topics that invoke some terrible responses, and therefore show an individuals true colors. I think we've seen a lack of humility, a boatload of arrogance, a touch of narcissism and a human that thinks they call the shots. Judgement time will come (if you believe in that kinda thing)

I think the issue is that the quinine is only working in mild to moderate cases, and even then the studies aren't 100% effective. I don't expect any study to be that good, especially when rushed. The mechanism of action of Quinine is somewhat unknown when it comes to augmenting the binding ability and affect on hemoglobin, there is a theory that the quinine can inhibit some processes of RNA replication, but I haven't dove into the specifics of it. We are almost all going rogue on the treatment of this and not awaiting the CDC recommendations, it's actually quite astonishing seeing/hearing from so many hatlcare providers and facilities that have almost no trust for the CDC right now. Whether right or wrong....for the record, I think this is a time to think outside of the box and let cookie cutter medicine fall by the wayside ?

It's quite unique, we're using ferritin levels as well to help guide us. LDH, procalcitonin, ferritin, C reactive protein and sometimes a DDimer (though I hate the test) we also see trends of low lymphocytes counts. It's almost like a flood the bloodstream with oxygen fix without all the positive pressure. The pathophysiology is interesting. Once you dust your brain off I'd love to hear your thoughts ?

Ok back to virus talk and medicine. It's amazing how much it's evolved over the course of a month, the previous thoughts of intubation are now being basically disproven, and there is evidence that intubation in its own right is causing the progressing ARDS. Several different factions and universities are doing many studies and the thought is now that perhaps it's not the lung damage itself or the pneumonia, but the inability for oxygen to Bind to hemoglobin due to iron displacement. The Peep and tidal volume we thought were helpful are the exact opposite in this mechanism of action. Granted, nothing is proven and there are several theories, but the number of theories that go against previous standard of care are pretty telling; and this also would help with the ventilator shortage. Many patients are oxygenating in the 60% with a simple nasal cannula or a hi flow mask and doing OK, we call them the "happy hypoxic". Previous thought has us intubating as soon as they are in the 70's for O2 but that's not the case here. I know this isn't is total layperson talk, but I find the science and theories being it pretty fascinating, and that is the part of Covid that makes my brain tick.

Heading to Saratoga lake tomorrow morning.... My plan is to find the "warmest" day next week and do the 56 mile bike course that the lake placid Ironman utilizes. I'm curious to see where I'm at in training and how far I have to go still (it's probably pretty far).

Blind faith is such a tough concept to grasp..... I've been to churches/temples/monestaries etc of all walks of faith. I have a respect for almost all of them (extremists of each faith being the ones I can't respect) Being a man of "science" makes it even more challenging, I know of many non believers that have converted to one specific religion. Has anyone here been a devout follower of one sect and then made a large change into another religion completely?
